Register with us

To register we require prospective patients to bring proof of address, your previous Doctor’s details, your NHS card, and ID We will ask you to fill out a GMS 1 form, which is your application to the list, (acceptance to the list is always at the discretion of the Doctor).

Please note that patients who are already registered with a local Doctor who wish to transfer onto our list; may be asked to attend an appointment with the Doctor prior to acceptance.

We will also ask you to complete a health questionnaire. You must attend your New Patient Screen to complete your registration. Book your appointment with Ms Lainey Fisher our Healthcare assistant.

We only register patients who live within our catchment area – see map of our area.

Please print the forms attached below and bring it to us with proof of identity in the form of photo ID and proof of address if you wish to register with our practice. If the form is not completely filled in then you will have to rebook your appointment.
